New State ID
The following documents must be brought in by the applicant to obtain a State ID (the following documents can not contain any lamination, tears or holes):
- Complete and print a State of Hawaii Identification Card Application (must be brought in with you)
- Original or certified copy of document(s) for proof of name and date of birth;
- Original or certified copy of document(s) for proof of legal presence;
- Two (2) documents for Proof of Residence Address (if your mailing address is different from physical address you will need to provide - one (1) document with mailing and one (1) document with physical address)
State ID term is for 8 years and the cost is $40.00.
MINORS
If applicant is a minor under the age of 18, the application must include a signature from a parent or by another individual in loco parentis of the minor who can provide proof of guardianship. A certified copy of the minor’s birth certificate is required to verify name of parent.
