Minors - Special Requirements
Age Requirement:
Minor must be 15 1/2 years of age at the time of application.
1. Birth Certificate
A certified copy of the minor’s birth certificate is required to establish acceptable signatory for parental consent as well as proof of legal name and date of birth. We do not accept any originals that have been taped, is torn or have holes punched in them. We must be able to read pertinent information on the document.
2. Proof of Social Security Number, Legal Presence, Residence
- Social Security Card
- Proof of legal presence
- Affidavit for Proof of Residence Address (If your post office box is your mailing address, you must also bring in a document with your physical address listed).

3. Acceptable Signatories for Instruction Permits for Minors
- If parents share custody, both parents must sign (parents married, separated or divorced with joint custody).
- Divorced parent with certified court custody papers indicating parent has legal custody may sign.
- If one parent is deceased, surviving parent may sign and must present deceased parent’s death certificate.
- Legal guardian(s) may sign with certified court documents.
- Foster parent(s) may sign with certified court documents. A minor applicant in foster care shall present verification of foster care on Department of Human Services letterhead signed by the Director or their designee.
- Next of kin may sign if both parents are deceased. Must provide death certificate for both parents.
- Responsible adult(s) may sign if both parents are deceased. Responsible adult(s) must be willing to assume financial responsibility of the minor and must provide death certificate for both parents.
- Legally emancipated minor may sign for himself/herself with court documents showing grant of emancipation.
- Adult spouse of minor applicant may sign with proof of age and marriage certificate.
4. Affidavit
Please download the Affidavit for Minors. Please note the Affidavit must be signed in front of a Notary Public unless signing in front of a Driver’s License Clerk.
Affidavit must be completed with a public notary or in front of a driver's license clerk or examiner.
If completed with public notary all information must be filled in PRIOR to notarization of document. We will not accept incomplete affidavits.
The names on affidavit must be reflected on the minor's birth certificate and if names do not match additional proof may be required.
Written exams are last issued at 3:00 pm
please be sure to check in by 2:30 pm to ensure you are given the test by 3:00 pm.
