If you have had a change of name, you will need to appear in person with the following documents:

1. State of Hawaii Identification Card Application - must be printed and brought in. We do not have a system to receive these electronically. 

2. Current State ID - this does need to be surrendered upon issuance of new license.

3. Present acceptable documents for name change (must be original or certified copy, cannot contain any tears, holes or erasures)

   • Certified marriage certificate
   • Certified divorce decree
   • Certified name change document                                                                                          

4. Two (2) acceptable documents addressed to you at your new address (If mailing address differs from physical address, you must provide a document for each address) Refer to acceptable documents list.

5. $6.00 fee (we currently only accept cash or check)

Your original license expiration date will remain the same. If you are eligible for a license renewal (six months prior to expiration) we recommend you apply for a license renewal instead of a duplicate license.
