Driver License Application:

Driver License Application must be printed and brought in. We do not have a system to receive these electronically. 

Affidavit for Proof of Residency:

Affidavit must be completed with a public notary or with a driver's license clerk or examiner and must be accompanied with a document(s) from the acceptable documents list. 

Affidavit for Minor - Instruction Permit:

Affidavit must be completed with a public notary or in front of a driver's license clerk or examiner.

If completed with public notary all information must be filled in PRIOR to notarization of document. We will not accept incomplete affidavits. 

The names on affidavit must be reflected on the minor's birth certificate and if names do not match additional proof may be required.
