Address Change By Mail 

If changing address by mail, the address change will be on your record only. 

To change your state identification card address by mail, provide the following:

•  Change of Address Form - must print & sign (We do not have a system to receive these electronically)
• Two (2) acceptable documents addressed to you at your new address (If mailing address differs from physical address, you must provide a document for each address) Refer to acceptable documents list.
