If you have had a change of name, you will need to appear in person with the following documents (the following documents can not contain any lamination, tears or holes):

Driver License Application - must be printed and brought in. We do not have a system to receive these electronically.

Current driver's license - must be surrendered upon issuance of new license.

Present acceptable document(s) for name change (must be original or certified copy, cannot contain any tears, holes or erasures)
- Certified marriage certificate
- Certified divorce decree
- Certified name change document

Two (2) acceptable documents addressed to you at your new address (If mailing address differs from physical address, you must provide a document for each address) Refer to acceptable documents list.

$6.00 fee (we currently only accept cash or check)
Your original license expiration date will remain the same. If you are eligible for a license renewal (six months prior to expiration) we recommend you apply for a license renewal instead of a duplicate license.
